Community Navigation

Our free Community Navigation Service supports people of all ages including seniors or adults with disabilities who may be at risk due to a critical or impending change in their life situation such as an upcoming eviction, facing homelessness, loss of financial income, or experiencing difficulties within their home setting such as abuse and isolation. 

Our Community Navigators work with individuals and/or caregivers to assess needs, develop an action plan and support them to stabilize their situation, access the necessities of life and help achieve goals of health, social, financial and housing self-sufficiency.

They do this by

  • providing information,
  • linking people to the most appropriate supports and services,
  • completing required forms,
  • making phone calls,
  • assisting with required technology,
  • supporting clients at meetings,
  • and coordinating support with other agencies.
